Entry form with Photos and rules

Do you know if its possible to creat 

a) an entry form where \

a) I ask for photograph 

b) accodring to the answer a different  script applies something like IF the

is

if I am an athlete I get the sport to register

if I am a volunteer I get the area where i want to help

You may create any type of form viz., contact forms, surveys, application forms, order forms, sign-up forms, RSVP, etc using JotForm by using its intuitive drag and drop method in the form builder.
