My account was unfairly suspended. I need to complete a form urgently. Please assist.

Your account was suspended due to your form asking for donations without proof of legitimacy or affiliation which is against our Terms of Use. For us to reactivate your account, kindly provide the following information:

1. Company name

2. Company website

3. Corporate email - you will have to use this on your JotForm account instead of a gmail domain.

4. Also, since you are asking for sensitive information, you will have to add T&C and Signature  widgets within your form once we reactivate your account to prevent your account from getting suspended again.
