Embed and/or Link Form to "My Google Website" website

    How do I create a form that automatically fills a contract (including signature) that I already have drawn up, and automatically sends the completed contract (in pdf or doc format) via email to the client (whose email will be in the form) and myself? I want this all to take place from a "My Google Website"?

    You can prepopulate fields in a form but not the Signature field.

    You can set up an autoresponder email in the form, which will send the form to the customer upon submitting the form.

    PDF attachments can be enabled in the autoresponder email settings to send PDF copy of submissions to the customers.

    If you want the form to auto submit, you can also setup a Global Countdown widget in the form and set up the time to auto submit.

    Hi,
    You have misunderstood my inquiry. I’m not asking how to perform prepopulated forms. My inquiry regards generating a .pdf file upon submission of a form. I think you may refer to the concept as “Fillable Forms”, whereas a client completes a form and upon submission, a .pdf file is automatically generated with the client’s form information filled within the .pdf file. I have pasted a link to the service agreement below, so that you can better understand what I want to accomplish. This service agreement is the exact .pdf file I wish to be generated upon completion and submission of the form by the client. In the service agreement, all the fields which need to be filled by the form are coded with the ${xxx} format ( with “xxx” being the information to be filled via the form). My signature and information is prefilled at the bottom of the agreement. The client signature (the blank signature field (1st signature field) near bottom of agreement) is the only field that needs to be filled. I need the client’s signature to generate in this field. So basically, I want to know if a form respondent can submit their signature via the form and the signature be filled in the proper space on the service agreement, thus automatically generating and sending a .pdf or .doc file to myself and the client upon submission of the form. If so, I would like to know the proper steps to making this happen. Thank you for the info regarding embedding the form to “My Google Websites”. Please advise. Thank you.

    My apologies for the misunderstanding. 

    There are two options:

    1. For this purpose you can use the Narrative fields widget, which allows you to create a similar form as you've shared - https://www.jotform.com/widgets/narrative-fields

    2. You can create a fillable PDF form using the shared document that you've shared, upload that form into JotForm and get the customer's information. They can then submit the form, which will generate a PDF copy along with the customer's information automatically transferred to your original PDF and send an email too.

    I think the second option would be more suitable for your requirement.

    Can I create a weblink for my form that opens to a webpage for the form instead of embedding it to my “My Google Website”? If so, please provide me with the proper instructions to do so. Please advise. Thank you.
