Import PDF

Hello.  I am trying to see if you guys have a solution for an agreement we utilize.  I would like it to have dynamic choices for us to fill out, like the date, address, service level, etc.  We have it in a klucky PDF, but uploading it wasn't very functional.  Thanks!

Just to confirm, you have tried to upload your PDF but it was not functional? Can you elaborate on this please.
