I am unable to log into my account and my form states it was disabled.

    This form https://form.jotform.com/200287874733058 is in violation of our terms. You cannot ask for credit card information per our terms.

    Second point is you will need to add the following widgets for compliance if you will be asking for SSN and ID#.

    1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

    2. Add a Signature widget where your users sign and agree to provide such sensitive information.
