How to include titles and other text so they come through in the submission form

    Hi. I would like to know how I can adjust my form (Registration Package) so all the text comes through in the submission. Right now there are some areas that only show signatures and dates but not the text that is above the signature area...  I attached several screen snaps that compare the form to the submission that comes through.  Perhaps I just missed where I can request that the text blocks / Titles of sections are included in the submission?    Also, I would like to know if we can change how the Guardian's name coming through where it now shows "Text Box A" and value... Perhaps I need to build it differently?  This is my first time using Jot Form so would appreciate the help.     Thank you for contacting our support team, I am more than happy to assist you with this issue.

    If you would like to have the text and headers appear in your submission data, you will need to enable that setting from within your submissions page. To do so, you will need to go to the submissions page of the form in question> then click on the Gear icon at the top right of the page > check the "Show headers and text" setting. Please use the screenshot below for reference:

    Could you help me with the area on the same form (Registration Package) when the submission is showing the "name / text box A" and value". see screen snaps.   Perhaps I can build these sections in a different way so the names don't come through in this chart form?  (There are a few sections where names are asked for within the text blocks).
