I have not received any forms since 5/05. I have been expecting several submissions these last few days, but none came in. One person emailed me last night saying she sumbitted an application through our website but never got a confirmation email (I don't see her application on Jotform) and another emailed me a couple of days ago saying that his application was not being accepted through our website. Please advise ASAP.

Are your forms embedded to a website? If yes, please share us the website url.
I also checked your account and I also cannot see new submission but when I tested one of your form I can now see the new submission. Please share us the form title so we can investigate further.

We have a few different forms. The one that receives the most submissions is The Veloz Group Internship Application, which is embedded to www.thevelozgroup.com/internships. We have been expecting submissions there and have not received any these last few days. Please advise.

I was able to find the problem , a required validation is preventing the form to be submitted.
I am forwarding this our developers right now

Ok, according to our engineers , the problem is that , you form has too many "_" that cause trouble on field's name on HTTP protocol. We tested the form by removing "____"s in the check box label, and everything is working normally now.
So I removed that label , and applied this custom CSS
I also added a div separator " _____________________ " into a Free text HTML

Your test submissions are #1816 and #1817. The last form we received previously was #1785. That means we are missing 30 applications. This is very important. Can you please ensure that I receive / recover them ASAP? Please advise.

They were just attempt to submit but never were submitted due to this problem. And since you have a hidden unique ID , this wil increment no matter if the submission went through.

So 30 people tried applying? How can I obtain these records? This is important.

Like I said, they were just attempts, so people were not able to submit the records. We in the support team have made at least 10 or 15 attempts. So I don't think it's 30 the number who did not submit their records. It's probably less than 10 people. If we consider that people would have tried to submit with no success at least twice

I got an email last night from someone who tried to submit an application using the form embedded at http://www.thevelozgroup.com/jobs and he said it won't sumbit. Please fix this problem and advise.

The problem is the same as above
You have set "________" in the check box label, which is preventing the form to submit.
Remove that label , and applied this custom CSS
If you want to still display such line, use Free Text HTML to the desired layout
