    I deeply apologize for this inconvenience. Your account was auto suspended by our anti-phishing system.

    Your form "Title Me" is suspected to be used for phishing as it asking for user's site password. FYI, JotForm forms cannot be used for authentication and is against our Terms of Use. Why do you need to ask for your user's password?

    For now your account will remain suspended. If you want it to be reactivated, this form needs to be deleted. Please let us know, we will delete this form and re-activate your account.

    Hi,

    Thanks for the email. Yes you can delete the form, it was going to be used to protect other forms so only my internal company staff can access via my website.

    I didn’t realise this was against your terms of use, sorry.
