Send unique PDF attachments per form

Hello! 

I have a form which I would like to send via URL in an email, but need each form to have a unique PDF embedded in it and sent to a corresponding email address. 

Can you please specify how unique is the PDF?

Will the PDF be included in the forms input field  or simply attached to the autoresponder email?

Will the form be prepopulated when you send it out to your users? 

How many unique PDFs will you be attaching?

What would be the basis for the different PDFs attached?

Please explain in full detail your needs so we can assist you efficiently. Also, please share a screenshot whenever possible.
