Theme not showing on published form

Hello,

I am building my form, and the theme I have chosen doesn't appear on the published form. 

I have tried to remove and reapply the them, and I am still getting the same result. 

I have attached a screenshot of what it looks like when I am building it and put the published link in the box below. 

I have applied a fix directly to your form.

It appears that the Page Image set for the theme is using an old link.

I have updated it, please feel free to let us know if you need any further assistance.
