Trouble creating report from the data collected

I need to produce a weakly report that show the reasons why participants failed the online screener.

This does not answer my question. I am trying to run analysis off of data that is collected within the form.

I need to produce a weekly report that shows the reasons why participants failed the online screener. I am trying to run an analysis off of data that is collected within the form.

Do you have any form field that can tell if the participant failed? Also, if you can provide a sample form or report that can achieve this, please share it so we can try to replicate it.
