Why my form is disabled?

    Hi,


    I am receiving a form disabled message - please advise as to why?

    I've checked your account and found that it is currently suspended. Your form was found as fake insurance.

    We understand this might not be the case with your forms, yet we had to act according to the general rule to comply with our Terms of Use: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    To activate your account, we will delete the guilty form. Or, please provide us the following information:

    1. The URL where your form is used.
    2. Your website of proof of affiliation to a legitimate institution.

    Also, we need you to change your registered e-mail to a corporate one to avoid getting flagged again by our phishing detector.

    Please, send these proofs to reviewers@jotform.com with Thread #2154469 as your subject. After reviewing your evidence, we'll let you know about the status of your account.
