Why does my account say suspended when I try to log in?

    Your account is suspended due to two reasons:

    1. The form "93368323465867 - Market Partner Form" was found collecting Social Security numbers which is sensitive information as per our Terms and cannot be collected via forms.

    However, if you are only collecting this from your customers and not from the general public, we can make an exception and activate your account provided that you add the below two mandatory widgets in the form:

     - Terms and Conditions https://www.jotform.com/widgets/terms-and-condition 

     - Signature https://www.jotform.com/widgets/signature 

    2. The forms "93368323465867 - Market Partner Form" &  "92456218667164 - Order Form Information" were found collecting credit card information using plain form fields i.e. in this case using the Address field. This is in violation of our Terms and PCI compliance.

    If you want to collect payments from your customers, you can use any of our authorized payment processors in your forms: https://www.jotform.com/integrations/category/payment-processing

    Hence, we will need to delete these credit card fields from both the forms in order to activate your account.

    Thank you for your reply.

    Unfortunately, for this kind of sensitive information, we are required to remove the fields so we are sure they are gone.

    In other to that, we need your permission to edit your form. Are you ok that we edit your form and remove the fields that my colleague mentioned? 

    As soon as we get your response, we will be able to proceed.

    I went ahead and deleted the fields on the suspected forms. I have now reactivated the account/username weinsteinstephie

    You may check the updated status when you log in again using the username at https://www.jotform.com/login
