Forms were all in the trash???

How did this happen? None of the links were working this entire weekend- I need to find out why we had to recover these from the trash where I certainly didn't put them, we paid to upgrade last Friday and we haven't had a good experience so far! A bunch of patients of ours were looking for these forms all weekend- how did this happen?

You may check the account history log for the time and IP address logged in and when the forms were deleted. Unfortunately, we cannot check your account history due to the HIPAA compliance.

Also, please make sure that your PC is not left unattended when you are away and locked. Also, please do not share your account password to anyone else.
