    Hi,

    It would be best to post your screenshot here, so that each of our support team members can take a look at your issue and add their inputs to help you find a more accurate solution to your problem. To post a screenshot, you may use one of the free image upload tools available in the net such as imageshack or photobucket. After uploading, post the url to the screenshot in this thread.

    We'll await your feedback.

    Your form looks good here compared to that screenshot so this is definitely a problem with your site's CSS. Is there any way you could upload the webpage in a test server?

    Hi Onika_g

    If you haven't tried it yet, please try to use the Iframe embed code below instead of the one you're using now in your website:

    <iframe allowtransparency="true" src="//form.jotform.com/form/11013740329" frameborder="0" style="width:100%; height:478px; border:none;" scrolling="no">

    </iframe>

    By using the iframe embed code, your website's CSS elements will not interfere with that of your JotForm form.

    Let us know how it goes. Thank you for using JotForm!
