My account was suspended because of phishing attempts

My account was suspended because of "phishing" attempts. We use a VPN and I believe that triggered the phishing label. Can you please reactivate the account?

I removed the guilty fields so I can re-activate your account back to good status. You can now use your forms properly.

Contact us again so we can guide you properly if you really want to collect SSN and Driver's License. 

We do want to collect those fields and were just creating the form in the free version and then will upgrade to the hipaa compliant version once the form is ready to go live and be used. Should I upgrade now to the HIPAA compliant version to continue creating the form?
Usually there is a testing phase and then actually activation of the form.
Assistance would be appreciated.

If you want to collect SSN and Driver's License, please add these widgets to your form:

https://www.jotform.com/widgets/signature

You will also need to provide us these:

1. Your company official website.

2. You need to use your company email and add it to your JotForm account. (You're already using it which is good)

After that, your forms can add and collect that sensitive information.

I hope this information helps.

Can we put the terms and conditions widget on our application but not the signature widget when collecting sensitive data? We do not require a signature on our forms as applicants are not entering into a contract with us.
