Can you please provide us more details on the problem? How not working do you mean... you could not submit the form?
I do not have the specific device for now, so, I could not really test your form on a Windows phone. But if you can provide me additional details about the issue, it would help.
I will also seek assistance from other support members who can test the form on the device.
As for desktop and even iphone, your form seems to work just fine when I tested it.
Please update us.

Thanks...so yes...we can't attach documents with this phone I mentioned. We try to browse to attach...but nothing happens. If you could try with other phones it would be great.

What is your current browser in your Windows Phone? Please try Google Chrome if it is supported.
