Why my form is disabled?

    I have been unable to successfully open a jotforms and would like to ensure it is up and running before I send it to anyone. 

    there are 2 forms and neither work. 

    would appreciate any help or direction you can appoint to me.

    Hi,

    I checked your account and you got suspended. Upon investigating your forms, I didn't see any violations against our terms so I activated your account back to good status.

    Please check your forms again and let us know if the issue still persists.
