On my mobile phone only, I enter zip code but get the "This field is required" message and I cannot submit the form

    I have another issue.  On my mobile phone only, I enter zip code but get the "This field is required" message and I cannot submit the form.

    I have tested a cloned version of your form using my android mobile device and I can't seem to replicate the issue at my end.

    May we know if you have embedded the form in to a webpage?

    I have cleared your form cache.

    Kindly test your form https://form.jotform.com/bfackrell/contact-form again and if the issue still persists, can we test a submission using your form?

    It's not embedded. I link directly your your server.

    I figured this one out.  It was because State was pre-populated so it doesn't recognize the entry with out manually entering it.

    Now I have a new issue (from my smart phone only).  If ill out the form and then select "File Upload" and browse to attache a file.  When I attache it refresh the from with all of my data gone and no file attached.
