I published but the preview link does not work

I have tried to open the form you have provided, and it loaded without any problems. 

If your form is enabled and ready for use, you can copy the form link and send it to everyone you wish to fill it. You can see the form link when you click on the Publish from the top menu.

Can you please tell us how exactly you've got to the page with the Form not found message, that way we would be able to provide you with better assistance.

I cut and pasted the form into the browser bar. Now I can’t access my account at all. It says it is suspended.

I have checked your account and it looks like everything is normal. Are you still unable to log in? If so, can you please provide us with the username of the account you are trying to log in.
