Form templates all link to product form

No access to other form templates.

I am looking to build a form using one of the templates but many that I click on link to one and the same product order template.

You can use all the templates shared there as they are publicly available.

Ok, see two screenshots and note URLs. In both instances I am trying to access the same form (Online doctor appointment). One is when I try to access specific form from your gallery, all fine.

The other one is when I actually attempt to build the form accessing the template from within my account, it then opens up a wrong template...

Hello @bhpinfo - Unfortunately I was unable to replicate the issue on my end. Please see my created form template here.  Please try to clear your browser's cache and cookies and see if this resolves the issue. I have also cleared your form caches.
