Template for a form like this?

    Hi there... I need to build a form for a client and I'm curious if there is a template similar to this.  Basically it's a lead generator where they submit their info and get a free PDF booklet on information, but in turn, we have their info for a lead.  Thanks.

    You can check out Lead Generation Form templates to see if there's something that you can use.

    Alternatively, you can just re-create the form by creating your own. How to Create Your First Web Form
