I have checked your account and it is currently on suspension status. The suspension is triggered by the following forms in your account.

- Qual o Defeito PC/Notebook ?
The forms have a Copyright Logo that is owned by a private company and is flagged by our system for suspicious phishing activity.
Jotform implements scripts that allow us to detect forms that are collecting sensitive information, our terms are clear regarding such type of forms.
Although we understand that this might not be the case with your form, we had to act according to the general rule to be in compliance with our Terms:
Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.
We will have to delete your form.
Or, you can provide us with the following so we can reactivate your account.
1. Your website.
2. Your corporate email address
Please send us an email at reviewers@jotform.com using your corporate email address. And, include in the subject this thread# 2168964. Briefly explain in the email the purpose of the form.
You can continue building similar forms in the future, but we suggest that you update the email address in your JotForm account to your corporate email address to avoid the same inconvenience.
