I am trying to link HR two forms together. These forms are an adaption from a single paper-based form that a manager would fill and sign alongside an employee returning to work after a sickness-related absence. As only one person can fill a form in at a time, I figured the best way to duplicate this on Jotform would be if the first form would be the employee's section and the second form would be the manager's section.
I would like the manager's form to have a pre-populated (and not editable) copy of the employee's completed form above the section the manager completes. So, Page 1 of the second form should be pre-populated and unchangeable. Page 2 should be what the manager completes. Ideally, the signature widget from the employee would also be pre-populated where it appears for the employee on the form.
I am struggling, however, to use the guide to make this happen. The forms are below:

Can someone please help make this happen? If this is something that JotForm can do, I have a significantly longer and more complicated form that I would like to apply this method to, which similarly required both the employee and manager to complete sections on what was originally a single paper form. A video tutorial would be useful.

Steps to generate the URL:
1. Generate the Form 2 URL with the Pre-populate app:

The generated URL would be in this format:
2. Next insert unique id of the fields of the first form into the words marked in bold.

Thank you very much! I am not familiar with apps in JotForm. How do they work and where would I find them? As I would like to build more forms with pre-populated data in the future, what would I do with it specifically to replicate this functionality?

Hi again.
I was having a play with this form and noticed two things:
1
. The second form automatically pops up for the person who submits the first form. I do not want this to happen. I want to second form with the pre-populated data to be sent to a different person; in this case it would be the manager whose email would be collected on the first form.2. The second form can be edited by the second person. I need this pre-populated data to not be editable after submission to protect the integrity of the original submission. How can I set this using the app?
