Account suspended

My account was suspended for phishing but im not sure why. can someone help me resolve this issue please.

This form https://www.jotform.com/200504226379148 was found on your account as suspicious of phishing activity because you're collecting user credentials.

Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly use something similar like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

I was trying to duplicate a survey i had built in survey monkey. i copied the link to the survey and put it in the form builder and thats what popped up. Is there a way to avoid this and actually copy the survey?
