Added fields (2) but these two fields are not showing in preview

    added two fields

    building name and Department name

    Neither shows in the form preview

    Thank you for contacting us.

    I checked your form and its Revision History Logs, it seems that there were no fields added on the history logs: How-to-View-Form-Revision-History.

    I also cloned your form to see if I could replicate the issue. I added Short Text Entry form elements for those fields and it is indeed on the preview mode or the changes are saved.
