The PDF form converter is messing up

I created a from using the PDF converter. The blocks keep moving on the page.

In the screen shot its the blue box that keeps moving.

I restarted the whole computer. Tried it in chrome, Firefox and edge. Sam problem.

Yes, the blue box moves sometimes but I try to put it back to the correct field. Is my screencast test correct? Are there any steps I need to do so I can replicate the issue?

No, It seems to be working on your screen test. I created a from prior to this one and had no problems. I don't know what the difference is with this one. I have deleted it and started over. I have duplicated the first form and just changed the pdf file. I have shared it and unshared it. I cant get it to stop moving and I don't know what the difference is.I would like to share the form with you if possible.

I have checked your video and it appears that the fields move away. Is this when only when you try to do it with the text field? I have tried to replicate the issue and here is a screencast for the same:

I know you have tried to clone the form and give it a try but can you please remove the text field and add the short text entry field for the fields such as Parent name in the text box and then try to connect them to the PDF?
