Our Form has stopped accepting responses to some questions

Hi There, 

We are struggling to get one of our forms working. 

it has been active for a few months but last night stopped displaying the option response to the main question.

I apologize for the inconvenience. I tested your form. It seems that it is working fine now.

Please note that there was a temporary slowdown earlier today and the widgets did not work. However, our backend team already fixed this. Everything should work fine now.

If the issue still persists, please provide the names of the fields, which do not work.
