when I get the email notification with the clients request, some parts of the answers are missing on the email
and an other problem:
when the client has filled out the form, the add of jotform is appearing? this should be not visible.Page URL: https://www.selva-apartments.com/request -

After cloning your form, I ran a few tests and found out that the problem is with the EMAILS Notifications settings. Some of the fields of your form aren't there.
To fix the issue, delete the current Notification and add it again. By default, all the fields will show up in the email. You can customize the name of each field as you desire.
This is what I get when I submit a test from the cloned form.

but I already tried to do this several times (look at he screenshot), but every time I left the form my changes are not saved?

Can you please make sure you click save before you exit out of the Notifications settings?

Hi,
The submission notification of a test sent from a clone of your form shows all the fields that you would like to set. See the screenshot below.
Kindly clone this form (https://form.jotform.com/200556233141948), test, and let me know if it works as you wish.
