My users are having issues!

Every now and again my users are not able to upload images into the form. This is causing major issues. Please take a look at my form and tell me if there is an issue. 

Is the issue related to the "Free Manual Order Submit" form?

I tested the standalone form and the upload field is working fine.

I saw another ticket from another user that has 3 other users experiencing the same issue. I personally cannot replicate the issue but 2 of my users are experiencing this today when everything had been mostly fine until today. This is an issue we have had in the past but it usually resolves fairly quick but this has been all day today.
