HIPAA: I received a blank submission

    Hello, i have a form submission that shows blank. I received an email that shows it was completed, but online everything shows blank. The same person filled up another form (different) that one I can see.

    This is the form, and the date is for Feb 24, 2020

    I need all this information since it is the medical intake

    To my understanding, it is showing blank after submission and not showing the Thank you page.

    Please try editing the Thank You page and then check if the issue still persists.

    Also, is the form embedded on a website? If yes, please share the link so we can check it too.

    That’s not what’s going on.. I have a submission which I got an email confirmation the fields were filled up, but when I check online, everything is blank.
    I got 2 emails from 2 submissions on the 24th (two different people) I can see one and not the other, and I can see the ones after so I know everything is working ok

    I can see that the form received two submissions on the said date. Could you please confirm which of these?

    Due to HIPAA restrictions, we're unable to view your form submissions as well so we cannot determine which of these two are you referring to.

    Once we receive your confirmation, we'll endorse this issue to our backend team for further investigation.

    I’m not really sure where to check that, but looking at the emails I got the confirmation at 4:10PM, the other one (the one I can see) I got the email at 6:05am
    So I’m assuming is the 5421
