Form field labels are set to the top but it is not displaying correctly on preview

    I have the label align of the fields set to "Top". They show fine in the editor, but appear all tangled up when I use the form link (see screen capture). Tried it in chrome and explorer with same results.

    Please help!

    The label settings needs to be re-applied. I did that for you. It's fixed now.
