I need to have an option in the form to duplicate fields multiple times

Hi there,

I need assistance creating a specific information collection feature. I need to have the option to have the following information have a (Add more) button so that multiple people can be filled in. Every time will be different. This entry will be mandatory for at least one entry. Example Below. 

Traveler name:

Cell number:

Office number:

Email:

Home address:

-Emergency Contact name:

Cell:

Alternative number:

Passport Copy:

(A button would then appear here in which the form taker could press in order to fill out the same set of details for another traveler. They could do this as many times as required.)

You can do this with our configurable list widget.

However, there is no upload section in the widget, so you have to ask it separately or ask the number only so you can use the Text input in the widget.

Hello Alex,

I checked your form, and you seem to have the widget already. I see it goes beyond the width of the form. You can make it responsive by doing the following option here:
