I can see that you only have two active forms on your account. If you're referring to this one: Black Maternal & Reproductive Justice Summit Registration Form 2020, I have checked that you have added a Square payment field but you haven't connected it with your Square account yet. This is the reason that keeps the form from being successfully submitted.
