Using JotForm with my EMR platform (Athena Health)

    I am looking for HIPPA compliant means of enrolling patients in my small medical practice. Ideally, I would like patients to be able to access new patient forms via hyperlink from my website. I would like to either 1) access the completed forms and personally upload them to my Athena EMR; OR 2) have them directly linked to the Athena EMR. 

    Would your business be able to assist with this and how?

    Jotform offers HIPAA compliance features, you may refer to this article to know more about our forms with HIPAA compliance features: https://www.jotform.com/hipaa/

    1) access the completed forms and personally upload them to my Athena EMR

    You can access the submissions on your forms through the submission page and you can also set up email notifications to get notified on every submission.

    I'm just not sure of how can you upload files to Athena EMR but you can download the submissions in Jotform and try to upload it to that software: How-to-Download-Form-Submissions-as-Excel-CSV-PDF

    2) have them directly linked to the Athena EMR. 

    Unfortunately, Athena EMR is not yet included in our existing list of integrations, so it's not possible to directly link this application to Jotform.
