Cloned form doesn't match the original

Hi, when we clone forms it is doing so in a strange format.  Instead of showing each individual question, it shows all of them at the same time in a different font.

Is something wrong?

I believe that you wish to clone a card form, and you have chosen the classic form layout.

If you wish to clone the card form, you need to select the Card format when you click the Create Form:

After that, you can choose the Import Form option.

If you already cloned form you don't need to clone the form again, you can change the form layout from the form Settings:

Please note that you need to click on How More Options and to scroll to the bottom to see the Form Layout option.
