    I've checked your form and it seems like your account was suspended due to the "ID CARD NUMBER", kindly note that in order to ask for this information it's needed to have the terms and signature elements on your form so you let the users know why you need to collect their information.

    What do you Méan by corporate email address?
    And I dont need thé signature form.it Will make things complicated for my
    customers.please explain further.

    A corporate email address is an email that reflects your business, for example, user@company.com

    Using an email address with a free provider such as "@gmail.com" or "outlook.com" will not help to validate your activity.

    Now, unfortunately, the change is required so you can collect the ID, otherwise you will need to remove that field from your form in order to avoid being suspended again.

    Ok I will add the signature part.But for the email I dont have a company
    website yet.When I will have one then I will be able to have corporate
    email.I hope i can use my gmail account for now.

    You may keep using the free email provider in the meantime, I would recommend you to update it as soon as you have a corporate email available.

    Ok thank you.
    I seem to have another problem I have upgraded my form and at
    the bottom it is still written create you own jot form it is free.I thought
    there should be no branding when I upgrade to silver.Please confirm.

    Find attached a reply message Jotform sent to one of my client after he
    submitted the jotform.I noticed that the branding is still at the bottom
