    Hi my form is automatically disabled kindly assist as soon as possible. Where can i send the URL?

    I have checked your account and see that there are two forms and they seem to be loading fine. Also, the account is active and good in standing. If you still notice any issue, please let us know. We will be happy to take a look again.
