Form showing disabled

    Dear Sir/Mam,

    I yesterday made a form on your website and for almost 2 days continuously i i hard worked to design that form for me. 

    But now when i am trying to open the page, a dialogue appears that this form is disabled, and even i am not able to login my JotForm account, it's says that Account suspended.


    Please help me clear about this, don't forget that i have given 2 continue days to design. Please dontd delete or disable my form.

    Hello @Intzar!

    I would like to apologize for the inconvenience that you are experiencing, I am more than happy to assist you with this issue.

    It seems that your account has been suspended for requesting private information in your Form-49A form. JotFrom does not allow the gathering of certain private information such as Credit Card details, Government ID numbers, Uploads of IDs or Credit Cards, etc. This specific form was suspended for all of the mentioned above.

    To have your account re-activated you will need to agree to our Terms of Use and allow us to delete the offending fields or the entire form. We will wait for your response to proceed.

    Additionally, if you would like to gather sensitive information such as this, you will need to do the following:

    1) Insert a Terms & Conditions widget into your form with your Terms of use

    2) Insert an E-Signature widget into your form so your respondents can acknowledge that they are providing sensitive information

    3) Change the email address on your account to one with a corporate domain

    I am authorising you to remove offending fields from my form. I agree with
    the terms and conditions specified by JotForm.
    Please do re-activate my form and my JotForm account too.

    I have removed the offending fields from the form. Please add the widgets mentioned above by my colleague in the form.
