Form Skipping Over Question

We have a form that was working perfectly fine, but is now skipping over the initial question and defaulting to a question that does not apply to all users (based on conditional formatting). It does not look like we have any "default" response set up for the question.

It seems there are conflicts with the conditions since you are using AutoNext feature.

Please try disabling conditions 1,2, and 5 then format conditions 3 and 4 like this one:
