Error when uploading files on embedded website

created a form through jotform however when we uploaded it to our website

it is not allow documents to be uploaded or when we press the submit button it  displays an error message

Finally, please let us know what exact error do you get when trying to submit the form? Also, do you get any error when uploading the document itself?
