I created a form and it was appearing and working fine on the site for a few weeks up until today. Now the form is not loading at all browsers.
I think it may have exceed allocated quota. Can you fix this form http://turk.jotform.com/form/10940113876 as soon as possible.

I have checked your form.
Your form is associated with a guest account and this will indeed reach an overlimit once you receive 20 submissions.
You will need to create a free account so that your submissions will be upgraded to allotted 100 submissions per month.
You can register by clicking My Forms on top of the page, then clicking Create a Free Account button on the left side panel, below your SSL submissions counter.
IMPORTANT: Please make sure that you have set an email address with your guest form before creating a free account. Use the same email address that you used in your guest form so it will automatically merge with your newly created account.
