Form disabled

    All of a sudden everyone of my forms got disabled.. I'm working on creating a opt in for my ebook and as well as a client intake form for my Credit Restoration Service I provide and I was hoping to tie everything together while I make my site I'm also working on.

    Apologies for the inconvenience. Your account was suspended due to your forms asking for sensitive information, including username, password, SSN and ID uploads which are all against our Terms of Use. I can also see you are using a copyright logo.
