I can see that you are having an Invalid Form URL Issue. It seems to me that this is related to Tablet Devices since when I test simulated it across a 3 different platforms only the iPad (Tablet) had an issue with the form.
Can you please try to see if it helps by changing your Form's Html Doctype to -
<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">
Windows 8 - Chrome: - Appears fine
iPad - Safari: - Form shows up as Invalid
Addtionally I also checked this on my iPhone because it is a mobile device but I didn't have any issues viewing your form there either so it mainly seems to affect Tablet or iPad Devices.
