My form isnt working the way it should and cant work out why

There are lots of options to tick, but they are not revealing themselves, I did three test tonight.. cant work out or fixed problem

I'm sorry but I do not understand what is the issue. 

I inspected the form and it has only two Skip page conditions, that are not necessary considering that your form has only one page.

Please explain in more details what exactly is not working and what are you trying to achieve.
