Account has been suspended

    Hello, 


    I am the Club Administrator for a not-for-profit Soccer Club in Ontario, Canada. My account has been suspended due to phishing as I had a form that was requesting information from employees in order to issue them a T4A. I then deleted the submissions for security purposes once the information was documented in our records. I really need my account reactivated as I have another form that has parents of athletes submitting documentation for a Scholarship and the deadline is fast approaching. With the account currently suspended many families are unable to submit the application and thus losing out on the Scholarship funding. I've submitted 2 requests already to have my account reactivated and have not received a response.

    Can someone please advise me of the status of my account?

    I've checked your account and it was suspended due to the "Social Insurance Number" field on your form https://www.jotform.com/form/200415731767252
