Account has been suspended

    I am the Club Administrator for a not-for-profit Soccer Club in Ontario, Canada. My account has been suspended due to phishing as I had a form that was requesting information from employees in order to issue them a T4A. I then deleted the submissions for security purposes once the information was documented in our records. I really need my account reactivated as I have another form that has parents of athletes submitting documentation for a Scholarship and the deadline is fast approaching. With the account currently suspended many families are unable to submit the application and thus losing out on the Scholarship funding. I've submitted 2 requests already to have my account reactivated and have not received a response.

    Can someone please advise me of the status of my account?

    I've checked your account and it was suspended due to the "Social Insurance Number" field on your form

    Kindly note that you need to edit your form in order to ask for this information, here are the elements you need to add: 

    1. A signature element so your users sign and agree to provide their information: 

    2. Terms text so your users can read and understand why you need their information: 

    Your account is active now so you can login and edit your form.
