Account wrongly suspended for phishing

hello, my account was wrongly suspended for phishing. I just used jotform to receive rental applications for my rental property and nothing more, i will be glad if this can be reviewed and my account restored soon.

Your account was suspended due to your form asking for uploads of Bank Details and IDs which are against our Terms Of Use.

You may ask for Bank Details and ID numbers in plain text field provided that you add a T&C and Signature widget within your form.
