    I just signed up and created by first form. The URL address is:

    http://www.jotform.com/form/11050219334

    When I tried to test it by performing a test submission, a pop up window appears and says the form is disabled.

    I am quite disappointed. This is my 3rd online form service I have tried.

    You have probably disabled your form. Here is how you can enable it back:

    1. Open your form on the form builder

    2. Click on Preferences button at the top

    3. Change "Status" from "Disabled" to "Enabled".

    Hello cihat,

    I noticed that your email address is connected to a guest account.  You probably have reached your quota limit so your form was disabled.  You will need to create a free account to increase your submissions quota.

    To create a free account, you can register by clicking the long orange button below the login fields.

    or the orange button on the left panel in your forms list page

    If you have other questions or concerns, please let us know.

    Thank you.

    @cihat

    It looks like the guest accounts under your email address are deleted due to phishing. Try creating a new account. Also, please be reminded that phishing is not tolerated. We will have your account deleted again and your ip address banned if you use Jotform for illegal internet activities.
