Account Suspended

    Good Morning,

           I received word this morning from one of our drivers that our forms are offline. When I attempted to login I get a message that says our account had been disabled due to a possible phishing attempt. We are a non profit that serves under the commission of the Nashville Fire Dept, and use your site for incident run summaries, shift change inspections, and volunteer applications. We decided to try the software before actually subscribing and committing, but up to this point we have been very pleased with the ease of these forms. I suspect this may be due to our volunteer application being created online which asks for a social security number so we can conduct a background check. We were just testing this form to see if it would work for us, but we had security concerns so we do not plan to use it on jotform. Please contact me so we can get the application form removed, get this resolved, and set our account up with a subscription.
