How can I get my PDF forms to upload without an error message

I get an error message stating "Unauthorized!"  I was able to do this with one form but no other ones.  I will be in appts until 12pm CST.  

I've am unable to attach the PDF that I actually created with your website that didn't save in the account but I fortunately did save on my desktop and could send you.

My website where I want the form located is given below (but doubtful that you need this although you can see that it did work for the NPP/HIPAA consent form).

Are you trying to attach a PDF file to your autoresponder or notification?

Where exactly do you get the "Unauthorized!" error?
